Sierra Pride Home Healthcare Agency is state-licensed and certified to provide therapy services, nursing care and medical social services. We also have staff who can take charge of some non-medical care needs that may arise in your given condition.
To ensure that you’re getting appropriate attention from our staff, we start by assessing your needs. Once we are able to identify your care requirements, we will design a care plan that outlines your health conditions and what treatment plans are most applicable. Along the way, we will coordinate with our physician and your family members. We closely communicate with you, your loved ones and your primary care physician while we provide you with quality home health services.

Mission Statement
Sierra Pride Home Healthcare Agency believes that Health Care is a basic human right. It is our desire to provide home care for people and families in their homes for the purpose of promoting, maintaining or restoring health or minimizing the effects of illness and disability, by providing licensed community health nurses or other staff as seen fit.
Our commitment is to safeguard human dignity, promote the potential for high levels of wellness in compliance with the applicable Federal, State and local laws and regulations, and the disclosure of ownership information to the best of our knowledge, based on our Quality Improvement Program, and compliance with acceptable professional standards and principles. Sierra Pride Home Healthcare Agency also believes that health care promotion is a collaborative effort with other health disciplines to provide consistent comprehensive care.

We utilize the interdisciplinary approach in providing services and work cooperatively with other community organizations and groups to provide comprehensive care.
